Comments:
An unannounced monitoring inspection was conducted on October 18, 2019 from 12:40 p.m. until 2:35 p.m. to review licensing requirements for family day homes. There were 2 children present, for a total of 7 points, in care with the provider. The records sample size consisted of 5 children?s records and 2 provider / household member records. Children were observed resting quietly at nap time, having an afternoon snack and playing with a variety of toys. A brief parent visit, diapering procedures and handwashing were also observed during today?s inspection.
The provider states that no medication is being administered to children in care at this time and that children are not routinely transported in a vehicle. Children play outdoors on the back lawn with direct supervision and close proximity of the provider. The provider reports that there have been no accidents or injuries requiring first aid or medical treatment incurred by children in care since the last inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of records and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to each child's attendance or within 30 days after their first day of attendance.
Evidence:
Five children's records were reviewed to include 3 currently enrolled children and 2 children who recently disenrolled from care.
The record for child #1, who began attending 9/10/18 and was no longer enrolled for care at this home, did not contain the results of a physical examination.
The record for child #3, who is currently enrolled and began attending 7/1/19, did not contain the results of a physical examination.
The provider stated that a physical examination is requested at the time that paperwork is completed but must not have been provided and was inadvertently overlooked.

Plan of Correction: The provider stated that she will request today that the parent of child #4 obtain this documentation to be placed in the child's record by the end of next week.
