Technical Assistance:
A discussion was held on ensuring children do not spend more than 30 minutes in a confined space, and after 30 minutes they have 60 minutes of floor, free time.

Comments:
An unannounced monitoring/renewal inspection was conducted today from 9:30am-11:00am. There were 7 children (19 points) supervised by 2 direct caregivers. The home, 2 caregivers records, 7 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed during circle time, singing about letters and colors and playing. Diapering procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-A
Description: Based on review of emergency drills, an emergency drill is not being practiced monthly.
Evidence:
An emergency evacuation drill was not practiced in the month of September.

Plan of Correction: I will document it every month.

Standard #: 22VAC40-111-960-G
Description: Based on observation, bottles were heated up in the microwave oven.
Evidence:
Caregiver, put a bottle in the microwave oven to heat it up.

Plan of Correction: We will not heat them up in the microwave any more.

Standard #: 22VAC40-111-60-A
Description: Based on review of 6 children's records, 2 are incomplete.
Evidence:
Child #1 does not have documentation of proof of birth. Provider insurance liability information sheet is not signed by parent.
Child #2 does not have an updated annual review documentation of emergency contacts from parent, last update 10/23/18

Plan of Correction: I will get the information from the parents.

Description: Based on observation, caregiver did not wash their hands with liquid soap and warm running water.
Evidence:
After diaper changes, caregiver did not wash their hands after completing diaper changes.

Plan of Correction: I will make sure hands are washed.

Standard #: 22VAC40-111-680-B
Description: Based on observation, children's hands were not washed with liquid soap and warm running water.
Evidence:
After having a diaper changed, children's hands were not washed with soap and water.

Plan of Correction: I will make sure all children's hands are washed after a diaper change.

Standard #: 22VAC40-111-690-D
Description: Diapering surface shall be cleaned with a solution of soap and water, patted dry, then spray with a solution of bleach and water and allowed to air dry.

Evidence:
Diapering surface was not cleaned or sanitized after a diaper change.

Plan of Correction: We usually use a rag and spray with lysol. We will use the 2 step process.
