Technical Assistance:
Technical Assistance was offered regarding central registry for a household member reaching the age of 14 years, new employee records.

Comments:
An unannounced monitoring inspection was conducted from 11:00 am to 12:10 pm. There were 5 children (16 points) present with 2 caregivers. The children were observed playing with toys, washing hands before lunch and eating lunch brought from home. 5 children's records, 2 caregiver records and 1 household member records were reviewed. Emergency drill records were current and there were no medications on site. Areas of non compliance are listed in the violation notice. Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain additional immunization records for children under the age of two, every six months.
Evidence: Child # 1's (16 months old) most recent immunization records were dated 7/2018. Additional immunization records were not obtained for Child # 1 after that date.

Plan of Correction: I will ask the parents to bring in the immunization records.
