Technical Assistance:
Discussed when cribs are occupied to re-position them away from objects within reach if children are able to pull up and stand in their cribs.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 8 children present (22 pts) with Provider and 2 assistants. When the inspector arrived this morning, the children were in two small groups. The younger children were playing on the floor with one staff. The older children were in the other program room with one staff and engaged with Legos, Mr. Potato Head and other toys and equipment. Walk through of the entire home was conducted today. (two levels). Observation of the crib room, bathroom and meal preparation area was made. 3 topical ointments were on site, no medications at this time and no identified food allergies. The older children transitioned to "class" where they discussed traffic lights, colors of the light and made a construction paper traffic light. Children were counting to ten in Spanish and singing songs. The Inspector observed some of the children putting on their own shoes and jackets with encouragement from staff to complete the task independently. Outside play time occurred towards the end of the inspection. A total of 7 children's records, all staff and 1 household record were reviewed. Previous violations from the May inspection were reviewed, and one repeated/uncorrected violation found. Other violations found today. Standard #: 22VAC40-111-210-A
Description: Based on staff record review, annual staff training was not completed.
Evidence: Staff # 3 received a total of 5 hours out of the 16 hours of annual training required.

Plan of Correction: This staff has enrolled in two classes in November and one class in December. Staff will obtain 16 hours of training each year.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the programming area, two areas were not maintained in a safe condition.
Evidence: The corner of the crib room has frayed carpet and areas of the sub flooring exposed. This remains repeated and uncorrected from the last inspection in May 2019. Peeling paint was observed on the interior wall of the crib room and also on the exterior wall of the playroom. Both these areas are accessible to children in care.

Plan of Correction: We are planning on replacing the carpet in the crib room and will repair the peeling paint. Pictures of completion will be forwarded to the licensing inspector.
