Comments:
An unannounced monitoring inspection was conducted today between the hours of 1:15 pm through 5 pm with the provider on site. There were 5 children (18 points) with the provider and assistant caregiver present. A sample of 5 children's files, 2 caregiver and all household member's files were reviewed. The children were engaged in the following: naptime, handwashing, snacktime , dancing, completing puzzles, and free play. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

Evidence: Child #1 began attendance on 8/16/18. The emergency information for Child #1 was last reviewed by parents on 8/16/18.

Plan of Correction: I will ask the parents to update the information.

Standard #: 22VAC40-111-120-B
Description: Based on record review, 1 of 2 caregiver records did not contain all required information.

Evidence:
The record for Caregiver #1 did not contain the name address and telephone number of a person to be notified in an emergency;

Plan of Correction: I will get the information from her today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 5 children's records did not contain all required information.

Evidence:
1. The record for Child #2 did not contain the address of the child's mother and father's place of employment.

Plan of Correction: I will ask the parents for the addresses today.

Standard #: 22VAC40-111-170-A
Description: Based on record review and provider interview, the provider did not obtain a current report of tuberculosis screening form for each adult household member.

Evidence:
Provider stated Household member #1 began residing in the family day home approximately 6 weeks ago. There was no documentation of a tuberculosis screening available for review.

Plan of Correction: I will take them to get it done.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review and provider interview, a criminal history check was not obtained for a household member over the age of 18.

Evidence:
1. The record for Household member #1 did not contain documentation of a criminal history check.
2. Provider stated Household member #1 moved into the family day home approximately 6 weeks ago and a criminal history check has not been completed.

Plan of Correction: I will take them to get it done.
