Technical Assistance:
The LI reviewed the Provider's renewal application that had been completed.

Comments:
An unannounced renewal inspection was completed today. The physical plant, 3 caregiver/household member files, 3 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:50-3:00pm. There were 3 children, totaling 6 points, under the supervision of 2 caregivers. Today, the children worked with arts and crafts (Thanksgiving themed), played outside, learned ABC's, did hand painting and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served pizza, chicken with mixed vegetables, mixed fruit and milk. The Provider is currently certified in MAT (medication administration training). There are no children with any medication needs at this time. Description: Based on file review, 1 out of 3 children's records does not have documentation of having provided all required information to the parents before the child's first day of attendance.
Evidence
1. Child #2 does not have a signed Provisions to the Emergency Preparedness Plan on file.
