Comments:

Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 1, 2019. The on-site inspection was conducted from 9:20a.m. to 11:40a.m. On this date, there were three children in the care of the provider for eight points. There were a total of five children enrolled in the program. There were five children's records reviewed. The records of the provider and one household member were reviewed. There were no medications being administered at the time of the inspection. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. There were an ample supply of age appropriate toys and equipment. Some items reviewed and observed were: infant care, story time, songs and large motor musical activities, playing with playdough, free play, lunch, emergency evacuation, shelter-in-place and lockdown drills, required posted information, daily attendance record, warm water temperature, refrigerator and freezer temperatures, first aid kit and emergency supplies, and nap time. Description:	Based on review of five children's records and verification from the provider, one child's record did not contain documentation that the child had received a physical examination. Evidence: 1. The record for Child #4 did not contain a record for a physical examination. Date of enrollment for Child #4 was 05/13/2019. 2. The provider confirmed the physical exam report had not been obtained.
Plan of Correction:	The provider has talked with the parent of the child and will obtain a physical examination as soon as possible.
