Comments:
An unannounced monitoring inspection was conducted today between 10:00 am and 12:30 pm. There were 8 children (21 points) present with 2 caregivers. The children were observed, working on puzzles, singing songs, reviewing colors and eating snack. Snack consisted of bananas and milk. The physical plant, attendance logs, evacuation logs, cribs, emergency plans, emergency supplies, children's records, caregiver's records and household member's records were inspected. The Family Day home was bright and had an abundant supply of age appropriate puzzles, books and art materials. There were no medications on site. Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to ensure that children's records are maintained and up to date for each child enrolled.

Evidence:
Child #1's record does not include documentation of an address for physician.

Plan of Correction: I will get the address from the parents and place in child's record.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver and household member's records, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
1) Caregivers #1 and 2's records do not have documentation of a current tuberculosis screening form. Caregiver #1 and #2's previous screenings were 11/2/2017.
2) Household member #1's record does not have documentation of a current tuberculosis screening form. Household member #1's previous screening was 11/2/17.

Plan of Correction: We will complete TB Screening on Monday, November 12, 2019

Standard #: 22VAC40-111-680-B
Description: Based on observation, the caregivers failed to ensure that children's hands are washed with liquid soap and warm running water before eating.

Evidence:
Prior to eating snack, the children did not wash their hands with liquid soap and warm water

Plan of Correction: I will ensure that the children's will wash their hands prior to eating.
