63.2 Child abuse and neglect
An unannounced monitoring inspection was conducted today from 8:45 to 10:45am with the provider. There were four children in care, ranging from 16 months to three years of age, for a total of 10 points. Children were observed during circle time- singing good morning, reviewing days of the week, talking about special days in November, reading a book and discussing the shapes they saw, reviewing the colors in Spanish, and talking about what they are thankful for. They then read books, did puzzles, washed their hands, and prepared for snack. Four children?s records, the provider's record, a household member's record, and a pet record were reviewed. The provider is current in CPR and First Aid certification. The provider reported that there is no medication for the children in the home at this time, but the provider is current in Medication Administration Training (MAT). The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. The last evacuation drill was documented as 10/17/19 and the last shelter-in-place drill was documented as 10/18/19. Standard #: 22VAC40-111-100-A Description: Based on review of four children's records, the provider did not obtain documentation of a physical prior to or within 30 days of first attendance for each child. Evidence: Child A and Child B did not have documentation of a physical in their records. Plan of Correction: The provider reached out to the family during the inspection to request a copy of the most recent well check.
Standard #: 22VAC40-111-180-A Description: Based on review of documentation, the provider did not obtain subsequent tuberculosis (TB) screening or testing at least every two years from the date of the initial screening or testing for staff and household members. Evidence: The most recent TB test/screening on file for the provider was dated 7/15/17 and the most recent TB test/screening on file for Household Member #1 was dated 9/2/17. Plan of Correction: We will get them done as soon as possible.
