Comments:
An unannounced monitoring inspection was conducted today from approximately 11:45 am to 2:30 pm. There were 12 children (30 points) in care of the provider and assistant. 5 children?s,2 staff records, 2 household records, caregiver training, children?s nutrition, supplies, emergency procedures and drills were inspected. The children were observed in free play activities, group center, musical movements, diapering and hand washing, lunch, and napping. There were developmental and age appropriate materials for all children in care.

There was an adequate number of staff certified in CPR and First Aid. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-750-D
Description: Based on observation and interview, the provider shall not used expired topical skin products.
Evidence:
1. The provider stated that she occasionally uses Desitin cream for Child #1 during diaper changing.
2. Child #1's Desitin cream ointment expired in 3/2019.

Plan of Correction: Discarded the expired one.

Standard #: 22VAC40-111-100-B
Description: Based on children's record review, the provider did not have documentation of a physical examination prior to child's attendance.
Evidence:
1. Child #2 (start date 4/02/18) did not have a physical within 30 days of the child's first day of attendance or within 30 days after the first day of attending.

Plan of Correction: We have gone over with all the kids records, requested with the parents if there was any missing record. Should have all these back by 11/05/2019.

Standard #: 22VAC40-111-60-B
Description: Based on children's record review, the provider did not obtain written authorization to administer prescription or nonprescription medication.
Evidence:
1. Child #2 (23-months-old) did not have written authorization to administer Desitin cream.
2. The provider stated Desitin cream is used occasionally during diaper changing.

Plan of Correction: We already got the permission/authorization form from the parent to use Desitin cream.

Standard #: 22VAC40-111-90-C
Description: Based on children's record review, the provider did not maintain additional immunizations, every six months for children under the age of two years.
Evidence:
1. Child # 1 (start date 3/04/18) and Child #2 (start date 4/02/18) every six months immunizations were not available for review.

Plan of Correction: We have gone over with all the kids records. Any missing record was requested from the parents should have all those back by 11/08/2019.

Standard #: 22VAC40-111-690-D
Description: Based on observation and interviews conducted, it was determined that the diaper changing surface was not cleaned or sanitized after use today.
Evidence:
1. After changing a child's diaper, Assistant #1 sprayed Lysol disinfected spray on the changing surface.
2. The surface was not cleaned with soap/water or a cleaning detergent solution; nor was it sanitized with a bleach/water solution or a sanitizing solution approved by the U.S. Environmental Protection Agency.

Plan of Correction: We start using water clorox bleache after licensing visit, for each and every time the diaper was changed.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on household record review and interview, family day home did not obtain central registry results for household members within 30 days of reaching the age of 14.
Evidence:
1. Provider was not able to provide documentation of household member #2?s central registry results.

Plan of Correction: We have started the process for central registry. Should be completed by the end of November 2019.
