63.2 Licensure and Registration Procedures

Background checks for one or more staff members were either missing or incomplete during today?s inspection. Licensee must provide documentation to the Licensing Inspector that the missing/incomplete background checks have been sent out within 10 calendar days of today's inspection. Failure to do so could potentially result in enforcement action.
One household members will be turning 18 years of age within the next year. The provider was reminded of the background check requirements for adult household members.

Today an unannounced Monitoring inspection took place between 10:10am and 12:20pm. There were 10 children present with the provider and 1 assistant for a total of 24 points within the supervision guidelines. The preschool aged children were observed practicing writing their names, learning letters and working on sight words; the infants and toddlers were signing songs and dancing. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance were observed during this inspection. Thank you for your cooperation. Standard #: 63.2-1720.1-B-3 Description: Based on record review and interview, the provider has not obtained an out-of-state central registry background check for a caregiver who resides outside of the state of Virginia.
Evidence: Caregiver #2 currently resides in the state of Maryland. The caregiver's file does not contain documentation of an out-of-state central registry background check for the state of Maryland. The provider has a print-out of the background check instructions in the caregivers file; it is documented that the background check was mailed on 9/6/2018 and again on 2/20/2019. The provider stated that she sent a third background check in June or July of 2019.Plan of Correction: The caregiver will send the background check again to a different address.
