Technical Assistance:
Storage of chemicals and insect spray in separate areas was discussed with the provider.
Updating the menu as substitutions are made was discussed.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:00 AM and departed at 1:15 PM. Five children, totaling 13 points, were in care. Two caregiver records and five children's records were reviewed. It was reported that no medication is administered to children in care and no children's injuries were reported.
An exit meeting was conducted with the provider prior to closure of the inspection.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based upon review of two caregiver records and provider interview, the provider has not obtained for each caregiverr a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. The record provided for caregiver 2 did not include the results of a tuberculosis screening.
2. The provider stated that the results of a tuberculosis screening were not obtained for caregiver 2.

Plan of Correction: The provider responded with the following:
Caregiver 2 will obtain a tuberculosis screening.

Standard #: 22VAC40-191-60-C-2
Description: Based upon review of caregiver records, the provider has not ensured that a child protective services (CPS) central registry search is obtained or continued employ is denied.
Evidence:
1. Results of a CPS central registry search were not in the record provided for caregiver 2 Caregiver 2 was on duty with the children during the inspection.
2. The provider verified that she was unable to locate the results a CPS central registry finding for caregiver 2.

Plan of Correction: The provider responded with the following:
Results of a new CPS central registry finding will be obtained if the original finding cannot be located.
