Technical Assistance:
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 11/7/2019. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

A renewed license cannot be issued until all outstanding background checks of household;d members have been verified.

Comments:
An unannounced renewal inspection was conducted today from 12:20pm-2:00pm. There were 5 children (15 points) directly supervised by the provider and assistnant. The home, 2 staff records, 5 children?s records, 1 household member records, evacuation drills, emergency supplies, and policies were inspected. The provider has completed a total of 20 hours of relevant training this calendar year. There were no medications on site today. Children were observed sleeping. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-90-C
Description: Based on record review, two of five children's records did not contain immunization record that have been updated every six months of children under the age of two years.
Evidence:
1. Child 2's (D.O.B. 8/1/2018) most recent immunizations were documented on 10/4/2018.
2. Child 3's (D.O.B. 6/10/2017) most recent immunizations were documented on 9/19/2018.

Plan of Correction: I will ask the parents to provide updated forms.

Standard #: 22VAC40-111-180-A
Description: Based on record review, one adult household member did not submit a current report of Tuberculosis screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
Adult 3's most recent Tuberculosis screening is dated 1/31/2014.

Plan of Correction: We will make an appointment immediately.

Description: Based on record review, the record for one adult household member did not contain a central registry background check conducted within the past five years.
Evidence:
Adult 3's record contained a central registry check that was conducted on 5/6/2013.

Plan of Correction: A new central registry check will be requested.
