63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on 11/07/2019 from 10:40 am - 11:40 am. At the time of entrance there were six children in care with the provider. The ages of the children counted for a total of 12 points. Children were observed during morning program free play. Records were reviewed for two children as well as the provider and one household member.
One violation was observed and is listed on the violation notice.

Violations:
Standard #: 22VAC40-111-500-C
Description: Based on observation, the provider failed to ensure that stationary outdoor playground equipment shall be placed at least six feet from the perimeter of other play structures or obstacles.

Evidence:
The back side of the outdoor swing set has been placed five feet from the fence and 42 inches from a plastic play structure.

Plan of Correction: This swing set will be moved over the weekend - weather allowing.
Children are not using this swing set at this time and will not use until moved.
