Technical Assistance:
Discussion was held on the topic of upcoming expiration date for a Tb screening.

Comments:
An unannounced monitoring visit was conducted today between the hours of approximately 8:30 am through 12:45 pm. There were 9 children (25 points) in care with 3 caregivers. A sample of 5 children's files, the provider's, 2 staff member's and 3 household members' files were reviewed. All three caregivers' required annual training hours had been met to this date. The children were engaged in the following: toy play, coloring, dance class, yoga time and reading stories with one of the assistants. Diapering, transition to lunch, lunch, bathroom breaks, hand washing, transition to nap time and nap was also observed. Lunch consisted of mac-n-cheese, cheese sticks, broccoli, bananas and milk. Cribs, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. There was no medication on site today. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-770
Description: Based on emergency supplies review, the provider did not have a portable battery-operated weather band radio as required.
Evidence:
1) Aportable battery-operated weather band radio was not available at the time of inspection.

Plan of Correction: The provider ordered a weather band radio online during inspection. It will arrive by Wednesday, November 13.

Standard #: 22VAC40-111-790
Description: Based on observation, phone numbers for 911 or local dial number for police, fire, and emergency medical responders; and the responsible person for emergency backup care were not posted in a visible area close to the telephone as required.
Evidence:
1) On the date of inspection (11/08/2019) required telephone numbers for 911, local police, fire, and emergency medical responders; and the responsible person for emergency backup care were not posted.

Plan of Correction: I will write the list and post it today.

Standard #: 22VAC40-111-800-B
Description: Based on document review and interview, the provider did not review the emergency plan at least annually and update the plan as needed.
Evidence:
1) The emergency plan did not documented a date of an update and annual review.

Plan of Correction: I will review the plan and post it on the board.

Standard #: 22VAC40-111-50-A
Description: Based on document review and interview, the written record of children in attendance was not accurate.
Evidence:
1) At the day of inspection, 9 children were in care in the family day home. The attendance record showed 10 children present.
2) The provider stated that they included the additional child because they thought they were coming but the mother messaged saying that they wouldn't because of a fever.

Plan of Correction: It was corrected during inspection. I will check myself the attendance everyday.

Standard #: 22VAC40-111-60-B
Description: Based on record review, not all files for children contained all required information.
Evidence:
1) The record for Child #1 did not contain a place of work listed and a phone number for the place of work for one parent.
2) The records for Child #1 and Child #2 did not contain a medical insurance policy number.
3) The record for Child #3 did not contain an address and phone number for one parent's place of work.
4) The record for Child #1 did not contain a phone number for a second emergency contact and Child #2 did not contain an address and phone number for a second emergency contact.
5) The record for Child #4 did not contain an address for one parent.

Plan of Correction: I will ask parents for the information.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain documentation of additional immunizations every six months for all children under the age of two.
Evidence:
1) The most current immunization report for Child #5 is dated 08/08/2017. An updated immunization report was due in 02/08/2018.

Plan of Correction: I will ask mom for the immunization report.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was not cleaned and sanitized after each use.
Evidence:
1) The assistants and provider were observed changing diapers to infants and toddlers and did not cleaned and sanitized the surface after each diaper change.
2) Although the caregivers utilize a paper barrier for each diaper change, licensing standards require for the surface to be cleaned and sanitized after each use, whether paper is used or not.

Plan of Correction: I will retrain my assistants and I will make sure we clean and sanitize every time.
