An unannounced monitoring inspection was conducted from 11:45 am to 12:30 pm. At the time of inspector's arrival there was 1 child, 3 yrs, for a total of 2 points, in care with the provider. The records sample size consisted of 5 children's records and 2 provider / household member records. The child was observed engaging in creative play, and interacting with the provider. The provider stated that no medication is administered to children while in her care and there have been no injuries. Areas of noncompliance are detailed in the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-111-60-B Description: Based on record review and interview it was determined the provider did not ensure that each child's record contained all of the required components.
Evidence:
1. The records for child #2, child #3, and child #4 enrolled 6/13/18 did not contain all of the information for one of the two required emergency contacts.
2. Provider confirmed that the records for child #2, child #3, and child #4 did not have the required emergency contact information.Plan of Correction: Provider will request the missing information from the parent.
