Comments:
An unannounced monitoring inspection was conducted today from 11:00am-12:45pm. There were 5 children (11 points) directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed coloring, gathering and participating in circle time, singing songs, reciting words in English and then Spanish and eating lunch.
Lunch served today was chicken nuggets, fries, carrots, apples, and milk. After lunch, children washed hands and were playing freely. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit did not contain all required items.

Evidence: The first aid kit is missing a second triangular bandage.

Plan of Correction: I will make sure to have a second triangular bandage in the first aid kit.

Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent the emergency contact information to ensure the information is correct annually.

Evidence: Child 1's emergency contact information was last reviewed on 06/16/2018.

Plan of Correction: I will have parents check the file today and sign.

Standard #: 22VAC40-111-60-B
Description: Based on record review, all chidren's records did not contain all the required information.

Evidence: Child 1 and 2 did not have start dates written down.

Plan of Correction: I will make sure to write the start dates on the paper.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider did not provide to parents in writing all required information prior to the child's first day of attendance.

Evidence: Child 2 is missing medication policy and information to parents.

Plan of Correction: I will provide parents with the information and have them sign.
