Technical Assistance:
Technical assistance today included physical environment, record keeping and menu accountability requirements.

Comments:
An unannounced monitoring inspection took place this morning from 11 am - 1 pm. There were 11 children in care with the provider and 2 assistants for 32 points. Standard #: 22VAC40-111-920
Description: Based on the posted menu, today's lunch was not accurately written.
Evidence:
The posted menu read that today's lunch would be turkey and zucchini, but the children had pasta and broccoli today. Substitute foods were not listed.

Plan of Correction: We will remember to write down any changes for parents to see before the meals are served.

Description: Based on observation during today's inspection, one baby's bottle was observed to not be labeled with the child's name.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-50-A
Description: Based on a review of attendance records today, some parents had signed in children in individual binders stored in another room, but this did not meet the requirements of a compiled and completed list of names for use in the event of an emergency .

Plan of Correction: We usually do a list. We will make sure attendance is done.

Standard #: 22VAC40-111-240-A
Description: Based on observation during today's inspection, there were some areas and furnishings that were not in a safe and operable condition.
Evidence:
Outdoors: There was splinter,ing on the outside of the door frame at child height, chipped paint on the wall and heavily soiled areas on the mats, surfaces of equipment and doors. The tot slide had a long string tied to it and a toy riding car was in poor condition, with a missing screen, leaving an exposed flexible wire of about 20 inches in length.

Plan of Correction: The string was removed from the slide during the inspection, and the riding car was placed in the trash. The splintery areas and dirty areas will be treated and cleaned.
