Technical Assistance:
Discussion on children?s records, hand washing and stairs.

Comments:
An unannounced renewal inspection was conducted today from approximately 11:15 am and 1:30 pm. There were 7 children (19 points) in care of the provider and assistant. A walk-through of the entire home was completed.
5 children?s, 2 staff records, caregiver training, children?s nutrition, supplies, emergency procedures and drills were inspected. The children were observed playing with toys on the floor, free play, group activity, lunch, and napping. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based upon review of 4 out 5 children records, the provide did not obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1(D.O.E 5/27/19), physical examination results were not available in the child?s record.
2. The provider verified that she had not received physical examination from parent for Child #1.

Plan of Correction: We have collected the documents of physical examination and information fro the parents and filled all the highlighted areas of 5 children records and taken the parent signature.

Standard #: 22VAC40-111-320-G
Description: Based on observation and staff interview, the provider did not obtain an annual inspection for fireplace, to verify that the device were properly installed, maintained, and cleaned as needed.
Evidence: The provider did not have documentation that fireplace had been inspected.

Plan of Correction: We appointed a contractor,she will be closing the fireplace permanently with hardwood, which were are not using at all.

Standard #: 22VAC40-111-430-B
Description: Based on direct observation, the provider was using a gate as a protective barriers at stair openings.
Evidence:
1. The provider was using an accordion gate to block the upstairs stair opening.
2. The gate was not mounted.

Plan of Correction: We appointed a contractor, who will be fixing the gate at stairs opening on Wednesday, 11/13/19.

Standard #: 22VAC40-111-680-B
Description: Based upon observations and staff interview, the provider did not ensure hands were washed with liquid soap and warm running water before eating.
Evidence: Two children were observed playing with toys before being placed at a table for lunch. The children were seated and served lunch. The provider did not wash the children hands before eating. The provider stated she washes the children hands after they eat.

Plan of Correction: We started cleaning the children hand wash before and after eating food.
