63.2 Child abuse and neglect

Comments:
An unannounced monitoring inspection was conducted today from 8:45am to 10:30am with the provider and one assistant. There were seven children in care, ranging from two to four years of age, for a total of 14 points. Children were observed playing in centers, reading books, playing in a sensory bin, during morning routine which consisted of singing and dancing, a bear hunt, and saying the pledge, and drawing a fall picture in their journals. Six children?s records and two staff records were reviewed. There are no household members at this time. The provider and assistant are current in CPR and First Aid certification. The provider reported that there is no medication on site at this time, however, the provider is current in Medication Administration Training (MAT). Lunch today will be peanut butter and jelly sandwiches, tomato soup, corn chips, oranges, and milk. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed and were in compliance. The emergency drill log, attendance, and menu were also reviewed and were in compliance. Standard #: 22VAC40-111-90-A
Description: Based on review of six childrens records, the provider did not obtain documentation that a child had been adequately immunized before attending the family day home. Evidence: Child A, with a start date of 8/6/18, did not have documentation of immunizations in their record.

Plan of Correction: The provider stated that she is sure she had the documentation but it must have been misplaced during a file review. If she does not find it by the end of the day she will ask the mother to give her another copy.
