63.2 Licensure and Registration Procedures

Technical Assistance:
Discussed the use of security cameras and clarified that security cameras are not required by licensing regulations. Discussed revising provider contract to include additional detailed information. Provided clarification regarding protective covers for the diapering surface, over the counter diapering ointments and annual fire inspections.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 9 children (32pts) with Provider and 2 staff on site during the inspection. The children were observed engaging with the staff on the floor with toys and equipment. The younger children were transitioning to morning nap. Observation of the diapering and hand washing procedures was made. Lunch time was observed. Staff were assisting with feeding and encouraging to eat. No food allergies identified at this time. Topical ointments, diapering area and remainder of the programming area was observed. Outdoor deck completed and a barrier at the base of the steps was observed. A total of 5 children's records, 3 staff records and CPR/First Aid certification were reviewed. This inspection began at approximately 9:30am and ended at 11:50am. Violations:
Standard #: 22VAC40-111-90-A
Description: Based on child record review, immunization documentation was not maintained.
Evidence: Child #2 began care on 9/9/19, immunizations were dated 9/17/19. Child #3 began care on 83/19, immunizations were dated 8/29/19. Child #4 began care on 9/16/19, immunizations were dated 9/19/19.

Plan of Correction: I obtained the documentation prior to care but replaced the immunization documentation with updated immunizations. I will maintain all child record documentation.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the family day home, one area of the home was not maintained in a safe manner.
Evidence: Two crib mattresses and one cot mattress had cracks and tears and are not longer non-absorbent.

Plan of Correction: I will order three new mattresses on Amazon and have them delivered tomorrow.

Standard #: 22VAC40-191-40-C-1-c
Description: Based on staff record review, background checks for one staff were not obtained.
Evidence: Staff #3 was hired on 9/24/19. The Central Registry background check on file was obtained from a previous employer. Background checks are not transferable.

Plan of Correction: A new Central Registry search request will be requested for this employee. The results will be forwarded the the State Inspector within 10 working days for review.

Standard #: 63.2-1720.1-B-2
Description: Based on staff record review, one background check was not obtained.
Evidence: Staff #3 was hired on 9/24/19. The fingerprint background check on file was obtained from a previous employer. Fingerprint background checks are not transferable.

Plan of Correction: This staff will complete the fingerprint background search and results will be forwarded to the State Inspector for review.
