Technical Assistance:
Discussion was held regarding record keeping, training and staff qualifications.

Comments:
A Subsidy Health and Safety inspection was conducted on 10/25/2019 to review supplemental health and safety requirements from approximately 8:00 am through 11:00 am. There were 8 children (22 points) in the direct care and supervision of the provider and a household member. A sample of 6 children's records, the provider's, 2 household members' records and 1 pet record were reviewed. The children were observed during breakfast, drawing, doing puzzles and free toy play. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-160-B
Description: Based on documentation review and interview, the provider did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1) A written attendance record was not available for review for the date of inspection nor any other dates
2) The provider stated that she forgets to take attendance.

Plan of Correction: I will make sure I have the attendance all the time.

Standard #: 22VAC40-665-230-E-1
Description: Based on document review and interview, the provider did not maintain a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.
Evidence:
1) The provider's most recent certification in CPR is dated 06/24/2017 and expired on 06/24/2019.
2) The provider stated that she did not realize it had expired.

Plan of Correction: I will take the class on 11/23/2019.

Description: Based on document review and interview, the provider did not maintain a current certification in first aid.
Evidence:
1) The provider's most recent certification in first aid is dated 06/24/2017 and expired on 06/24/2019.
2) The provider stated that she did not realize it had expired.

Description: Based on document review, evacuation procedures were not practiced at least monthly.
Evidence:
1) Evacuation procedures drills were documented for the months of January through March of 2019. No other drills were documented for the months of April through October.

Plan of Correction: I update the fired drills.
