Technical Assistance:
The provider must submit two fully completed reference checks to be included with the initial application.

Comments:
An unannounced full initial inspection for licensure was conducted today. The provider is not currently caring for children. This inspection was completed based on a review of the physical plant, a review of all required administrative records, a review of all required emergency preparedness items, observations of the provider's supplies for children, and a review of the provider's planned programming activities.

The child care area is clean and supplied with a variety of age-appropriate materials.

Violations:
Standard #: 22VAC40-111-530-A
Description: Based on observations and provider interviews, it was determined that the provider is not equipped to provide linens of a top and bottom cover for the children's rest mats.
Evidence:
The provider does not have sheets and blankets, or a top and bottom cover, for the children's rest mats.

Plan of Correction: I will purchase a sheet and blanket before children enter care.
