Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the status of the renewal application. The Provider stated that the application has been mailed. Discussion was also held on the topic of a substitute provider. All substitute providers must have the same qualifications and all background checks would need to be in place (just as originally required for the actual Provider). Reference checks, Sworn Disclosure Statement, Orientation Training, and CPS & First Aid certifications would also need to be documented and on file with in the Family Day Home.

Comments:
The following inspection took place between the hours of 10 am through 12 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Falls Church, Virginia. At the start of this unannounced renewal inspection there were no children in care for today. A total of 1 child?s file, 1 staff file, and 7 household member?s files were reviewed. All previous violation were corrected except for one from the last inspection conducted on October 22, 2019. If you should have any future questions, please call or send an e-mail to charles.perkins@dss.virginia.gov. Thank you.
