Comments:
An unannounced monitoring inspection was conducted today from 11:20am-12:30pm. There were 11 children (22 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, being spoon fed lunch, preparing for nap, and napping.There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, children were being allowed to eat while walking and standing.

Evidence: Caregiver 2 was observed feeding two children as they were standing and walking around.

Plan of Correction: We will make sure to feed children while the are sitting at a table.

Standard #: 22VAC40-111-180-A
Description: Based on observation, the provider did not obtain for each caregiver a current Report of Tuberculosis screening form every two years from the date of the most recent screening.

Evidence: Household member 1 has a tuberculosis test conducted on 3/4/17 and has not been updated.

Plan of Correction: We will make sure to get the tb test.

Standard #: 22VAC40-111-320-B
Description: Based on observation, child-resistant protective covers were not installed in all unused electrical outlets accessible to children under five years of age.

Evidence: In the upstairs living room, two unused electrical outlets did not have child-resistant protective covers in place. Provider stated children typically stay upstairs during the early morning while waiting for the second caregiver to arrive.

Plan of Correction: Covers will be placed in the outlets.

Standard #: 22VAC40-111-430-C
Description: Based on observation and evidence, children over the age of two had access to stairs.

Evidence: Provider stated children typically stay upstairs during the early morning while waiting for the second caregiver to arrive. The stairs leading to the second floor and the stairs leading to the lower level child care area did not have a gate preventing access to children.

Plan of Correction: I will put a gate.
