Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:15pm. There were 7 children (23 points) directly supervised by 2 staff. The home, 2 staff records, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, coloring, singing songs, and doing action songs. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-960-B
Description: Based on observation, an infant who could not hold their own bottle was not picked up and held for bottle feeding.

Evidence: Child 1, 6 months old, was observed drinking a bottle and blankets were placed underneath both the child's arms to prop up the child's arms to hold the bottle. Caregiver stated that the child cannot hold the bottle for the entire feeding therefore the child was placed in the crib with the blankets to help the child hold their bottle.

Plan of Correction: We will no longer prop up bottles to help the children.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not ensure children's records contained all the required information.

Evidence: Child 1 and 2 are missing start dates. Child 1 is missing two parent work phone numbers.

Plan of Correction: When parent picks up today, I will have them update their record. I will add the start dates to the children's record.

Standard #: 22VAC40-111-530-F
Description: Based on observation, soft bedding was used around infants while in a crib.

Evidence: Child 1, 6 months old, was observed in a crib with one small comforter and two blankets placed underneath the child's arms and body to help the child drink their bottle.

Plan of Correction: The two blankets were removed from the child. Then the child was removed from the crib.
