Technical Assistance:
Technical assistance was offered regarding: renewal of license and background checks.

Comments:
An unannounced monitoring inspection was conducted from 10:45 am through 12:40 pm. There were 11 children (40 points) with 3 caregivers present. A total of 12 children's records, 3 caregiver's records and 1 household member's record was reviewed. The children were engaged in floor play with age appropriate toys, washing hands before lunch and eating lunch. The lunch menu included vegetable soup, pasta, chicken, pears, vegetable crackers and milk. Positive caregiver to children interactions were observed. Violations:
Standard #: 22VAC40-111-120-A
Description: Based on record reviews and staff interview, the provider did not obtain documentation of two written references for a caregiver (CG) at the family day home.
Evidence:
1. CG# 2's first date of employment was 2/4/19.
2. CG # 2's record only had one written reference check.
2. CG # 1 stated that she did not obtain the second reference check.

Plan of Correction: The second reference will be completed and maintained in the record.

Standard #: 22VAC40-111-60-A
Description: Based on staff interview, the provider did not maintain a complete record for each child enrolled in the family day home.
Evidence:
1. Caregiver (CG) # 1 stated that Child # 1 started care on 11/11/19.
2. CG # 1 stated that she has provided the parents with the paperwork for Child # 1 but they have not returned them.

Plan of Correction: I will remind the parents to bring in the paperwork.

Standard #: 22VAC40-111-90-C
Description: Based on records review, the family day home did not obtain additional immunization records for children under the age of two years, every six months.
Evidence:
1. Child # 2's (20 months old) most recent immunizations were dated 11/2018.
2. Child # 3's (14 months old) most recent immunizations were dated 12/2018.
3. Additional immunizations for Child # 2 and Child # 3 were not obtained after those dates.

Plan of Correction: I will ask the parents to bring in the immunization records.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the family day home did not obtain subsequent tuberculosis (TB) screenings for adult household members (HHM) every two years.
Evidence: HHM # 1's most recent TB screening was dated 5/3/17. It expired on 5/3/19.

Plan of Correction: I will obtain a new TB screening for HHM # 1.
