Technical Assistance:
Discussed the importance of maintaining compliance during the Conditional period of the license. Discussed the requirement that each infant is required a crib. When an additional crib is purchased, please maintain all manufacturer's paperwork on file.

Comments:
An unannounced non-mandated inspection was conducted this date to evaluate compliance during the first two months of licensure. A total of 5 (14pts) children were in care, Provider's one child was included in the total. Observation of the home, outside play area and screened porch area was made. The children were engaged with the provider as they completed circle time. The children sang songs in Spanish, counted numbers and reviewed the days of the week and the weather. Previous violations from the Initial inspection were reviewed, and some repeated/uncorrected violations were found. This inspection began at approximately 9:20am and ended at 11:15am. Standard #: 22VAC40-111-750-A
Description: Based on review of onsite topical ointments, requirements were not met.
Evidence: Child #2 and #4 have topical ointments on site. Written parent authorizations were not obtained. Both topical ointments were not labeled with the child's name.

Plan of Correction: I will use the model form for written authorization and have both parents sign today.

Standard #: 22VAC40-111-870-G
Description: Based on observation of the kitchen area, a table and chair tray was not sanitized.
Evidence: The table located in the family room and the high chair tray in the kitchen was found with dried food. This is repeated from the Initial Inspection in September.

Plan of Correction: I will sanitize directly after the children finish eating.

Standard #: 22VAC40-111-920
Description: Based on observation of the home, a menu was not posted or provided to the parents.
Evidence: A current one week menu was not found posted and is not provided to parents. This is repeated from the Initial Inspection in September.

Plan of Correction: I will use the USDA menu and post this.

Standard #: 22VAC40-111-930-C
Description: Based on observation of the kitchen area, eating utensils and dishes were not cleaned.
Evidence: The kitchen sink and kitchen counter area had dishes and utensils that were not washed. The kitchen sink was full of dirty dishes.

Plan of Correction: I will maintain the dishes and wash them frequently to prevent the sink from filling up with dirty dishes.

Standard #: 22VAC40-111-960-E
Description: Based on observation of the kitchen area, bottles were not labeled and dated.
Evidence: Bottles for Child #1 and #2 were not labeled and dated.

Plan of Correction: I will label and date each bottle every day.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, one out of four records reviewed was not complete.
Evidence: Child #2's record does not include one parent's work address and insurance policy number.

Plan of Correction: I will have the parent complete this at pick up today.
