Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 7 children, totaling 21 points. Children's records, background checks, caregiver training, the physical plant, children's nutrition, child care activities, interactions, and emergency preparedness items.

The children were observed playing with PlayDoh, dancing to music, creating art projects, and preparing for lunch. Pleasant interactions between caregivers and children were observed throughout this inspection.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on a review of emergency preparedness items, it was determined that the provider did not review the emergency preparedness plan annually.
Evidence:
The provider did not complete an annual review of the emergency preparedness plan. This plan was most recently reviewed by the provider on 1/23/18.

Plan of Correction: I will remember to review the plan with my assistants today.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
Child record A does not include the mother's work address.

Plan of Correction: I will ask the parent for the information.

Standard #: 22VAC40-111-170-A
Description: Based on a review of adult household member health records, it was determined that one adult household member has not obtained a TB Health Screening.
Evidence:
Household member B turned adult age on 10/29/2018. She has not obtained a TB test.

Plan of Correction: I will make sure she gets a TB test.
