Comments:
An Unannounced Renewal Inspection was conducted on November 19, 2019 from approximately 9:10 am until 11:15 am. There were three children in care with one provider, totaling 10 points. The sample size of records reviewed included three children's records, one household member record and one provider record. There are no medications being administered. The results of the inspection were reviewed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on documentation review and provider interviews, the provider did not ensure the emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

Evidence: The provider confirmed there was not documentation of drills conducted in July, August and October .

Plan of Correction: The provider will document drills.

Standard #: 22VAC40-111-60-A
Description: Based on a review of 3 children's records, the provider did not ensure to maintain an up-to-date record at the family day home for each enrolled child.

Evidence: The records for three children in care were missing the following information. The provider confirmed the information was missing.
1) The record for child #1 was missing the house number or apartment number and zip code for one emergency contact listed.
2) The record for child #2 was missing the employment number for both parents listed and the insurance policy number.
3) The record for child #3 was missing the employment number for one parent listed, the insurance policy number, first date of attendance, house number or apartment number, street name and zip code for one emergency contact listed.

Plan of Correction: The provider will get the records updated.

Standard #: 22VAC40-111-680-A
Description: Based on observation and provider interviews, the provider did not ensure to wash their hands with liquid soap and warm running water when required.

Evidence: The provider was observed serving snack, cereal and peeling oranges and did not wash their hands prior. Caregivers are required to wash their hands before preparing and serving food.

Description: Based on observation and provider interviews, the provider did not ensure children washed their hands with liquid soap and warm running water when required.

Evidence: A child was observed during diaper change and their hands were not washed after diapering. Two children were observed eating snack and they did not wash their hands before eating.

Standard #: 22VAC40-111-690-D
Description: Based on observation and provider interviews, the provider did not ensure the diapering surface was cleaned and sanitized after each use.

Evidence: A child was observed during diaper change and the diaper pad was sprayed with a disinfectant only and the diaper pad is required to be both cleaned and sanitized.

Plan of Correction: The provider will make sure they start doing that now.
