Evidence:
1. Upon arrival at the family day home, the provider had medication on a tray in the living room.

Plan of Correction: The provider placed the medication in a locked place during the inspection.

Standard #: 22VAC40-111-100-A
Description: Based on record reviews, the provider failed to obtain documentation of a physical exam.
Evidence:
1. The inspector reviewed six children's records. One of six records reviewed (child record #1) did not contain documentation of a physical exam. Child's date of enrollment was 06/10/2019.

Plan of Correction: The provider will ask the child's parents to provide a copy of the child's physical.

Standard #: 22VAC40-111-70-A
Description: Based on record reviews, the provider failed to obtain parent's signatures for all of the family day home's policies.
Evidence:
1. The inspector reviewed six children's records. One of six records reviewed (child record #4) did not contain the provisions of the home's emergency preparedness and response plan.

Plan of Correction: The provider will give this document to the child's parents and as that they review and sign.

Description: Based on record review, the provider failed to obtain proof of identity and age.
Evidence:
1. The inspector reviewed six children's records. One of six records reviewed (child record #2) did not contain documentation of reviewing child's proof of identity and age.

Plan of Correction: The provider will ask the child's parents to provide this information for the child's record.

Standard #: 22VAC40-111-320-C
Description: Based on observation, the provider failed to have all electrical devices unplugged near a water source.
Evidence:
1. The inspector observed the provider's bathroom. There was a curling iron plugged in near the bathroom sink.

Plan of Correction: The provider unplugged the curling iron and placed an outlet cover in the outlet during the inspection. The provider will place the curling iron in a cabinet.
