Technical Assistance:
Discussed with provider to include all food items served in the posted menu.

Discussed required background check requirements for a teacher to e utilized as a backup caregiver.

Comments:
An unannounced renewal inspection was conducted today from 11:00am-12:30 pm. There were 7 children present (17 points) directly supervised by the provider and an assistant. The home, 3 staff records, 5 children?s records, 2 household member?s records, caregiver training, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today in accordance with the provider's medication policy. Children were observed playing freely and eating eating lunch. Diapering and handwashing procedures were also observed. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-750-A
Description: Based on record review and provider statement, written parent authorization for the administration of topical ointments was not present on the date of inspection.
Evidence:
A tub of topical ointments for use by children in care was observed. When asked for the authorizations, the provider said that she does not have a written authorization.

Plan of Correction: My inspector is emailing me the form and I will have parents complete the authorization.

Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not informed in writing of some of the provider's policies required.
Evidence:
The records for Children 1, 2, 3, 4, and 5 did not contain documentation that parents were informed, in writing of the following policies: Schedule of fees and payment plans; Check in and check out procedures; Amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider; Parental notifications required in 22 VAC 40-111-650; Policies for termination of care.

Plan of Correction: I will complete the missing sections of the Information for Parents form and have the parents sign the updates sections.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper changing surface was not sanitized after each use.
Evidence:
Adult 1 was observed to change a diaper and cleaned the changing pad with a wipe immediately following the diaper change. This does not meet the definition of "sanitized". "Sanitized" means treated in such a way as to remove bacteria and viruses from inanimate surfaces through first cleaning and secondly using a solution of one tablespoon of bleach mixed with one gallon of water and prepared fresh daily or using a sanitizing solution approved by the U.S. Environmental Protection Agency. The surface of the item is sprayed or dipped into the sanitizing solution and then allowed to air dry.

Plan of Correction: I will use a disinfectant spray in the future.

Standard #: 22VAC40-111-690-E
Description: Based on observation, soiled diapers were not disposed in a storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
Evidence:
A soiled diaper was disposed in a foot operated container in the bathroom. The bathroom door was opened with the caregiver's unwashed hand prior to disposal.

Plan of Correction: I will leave the bathroom door open when changing diapers so that it is unnecessary to open it.
