Comments:
An unannounced monitoring inspection was completed on 11/19/2019. There were no children and one provider present during the inspection. The inspector reviewed 3 children's record, one provider record, one household member record, and 2 pet records during the inspection. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed infant care and adult household members with the provider. Violations:
Standard #: 22VAC40-111-800-B
Description: Based on document review and discussion, the provider failed to ensure that the emergency plan shall be reviewed and updated at least annually as needed and the provider shall document in writing each review and update to the emergency plan.

Evidence: The emergency plan had been most recently updated on 06/01/2018.

Plan of Correction: The plan was updated during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review and discussion, the provider failed to ensure that each child's record shall contain all of the elements as required by the standards.

Evidence: The record for Child #1 did not contain the following information: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there was only one designated person provided) and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.

Plan of Correction: The provider will get the information from the parent.

Standard #: 22VAC40-111-90-C
Description: Based on record review and discussion, the provider failed to obtain documentation of additional immunization for a child who is not exempt from the immunization requirements.

Evidence: The record for Child #2 did not contain documentation of additional immunizations once every 6 months for children under the age of two years; the most recently dated immunization record in the record was dated 11/2018.

Plan of Correction: The information will be requested and gotten from the parent.
