An unannounced monitoring inspection was conducted today. There are 5 children present (10 points) with 2 caregivers. 6 adult and 5 children's records were reviewed. See violation notice for missing items. Children were observed, paying, dancing, coloring and listening to stories. First aid supplies, emergency supplies, emergency evacuation drills and bathroom supplies are all in compliance.

Standard #: 22VAC40-111-180-A Description: Based on a review of records, one adult record does not contain documentation of a subsequent TB screening within 2 years.
Evidence:
The TB screening results available for review are dated 8/4/17 (expired). (Adult A)Plan of Correction: The screening was done, I just cannot locate the paperwork. I will find the paperwork or get another copy of the results.
