Technical Assistance:
Discussed the Auvi-Q training that is available to complete online. Reminder to complete sworn disclosure statements soon.

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 4 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 12:30-2:00pm. There were 13* children, totaling 41 points, under the supervision of 3 caregivers. * See Violation Notice. During the inspection, children were observed being fed bottles (infants), enjoying free play, watching an educational video, cleaning up for and resting at nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served cheese pizza, apple sauce and milk. Standard #: 22VAC40-111-920
Description: Based on observations and interview, the Provider did not list any substituted foods on the posted menu.
Evidence
1. The posted menu had the following listed for lunch today: macaroni and cheese, yogurt, carrots and milk".
However, when the Provider was asked what was for lunch, she stated they were served "cheese pizza, apple sauce and milk".

Plan of Correction: The Provider will list substituted foods on the menu.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 2 out of 5 children's records do not have all required information on file.
Evidence
1. Children #3 and #4 do not have the work address for both parents listed on the registration form.

Plan of Correction: The Provider will ask the parents to complete this.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 5 children's records does not have additional immunizations every six months up to 2 years.
Evidence
1. Child #2 is 2 years old. The most recent immunization record was completed when the child was 15 months old.

Plan of Correction: The Provider will ask the parents for this.

Standard #: 22VAC40-80-120-A-4
Description: Based on observations, the FDH failed to comply with the terms of the license,
Evidence
1. The license has an allowable capacity of 12 children. Today, there were 13 children in care.

Plan of Correction: The Provider will make sure that no more than 12 children are in care at one time.
