Discussion was held on the topics of current enrollment, current staffing and a discussion on diapering changing surface. Please submit future renewal applications prior to expiration date.

Inspection Summary
An unannounced monitoring inspection was conducted today from approximately 11:25 am and 1:35 pm. There were 4 children (19 points) in care of the provider and assistant. A walk-through of the entire home was completed. Upon arrival, the provider was taking the children to the playground.

5 children?s, provider record, 3 household records, caregiver training, children?s nutrition, supplies, emergency procedures and drills were inspected. The children were observed playing with toys on the floor, free play, group activity, lunch, and napping. Diapering and hand washing were observed.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based upon review of 3 out 4 children records, the provider did not obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (D.O.E 8/02/2017), physical examination results were not available in the child?s record.
2. The provider verified that she had not received physical examination from parent for Child #1.

Plan of Correction: I have given Child #1 parents the form and am waiting for them to return it

Standard #: 22VAC40-111-50-C
Description: Based on observation, the provider failed to ensure that all children's records were kept confidential.
Evidence: The inspector observed that the supplemental page that contains the children's name from the previous inspection (5/20/2019) was posted on the parent board. Supplemental page states that it is not to be posted.

Plan of Correction: I have removed the confidential form from the board

Standard #: 22VAC40-111-680-A
Description: Based on observation, the provider did not ensure that their hands were washed with soap and warm running water after diapering a child or assisting a child with toileting.
Evidence: The provider changed two children diapers on a diapering changing surface on the floor. The provider did not wash hands after the diapering the two children.

Plan of Correction: Since the observation, I have been washing hands after every diaper change

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure that children's hands were washed with soap and warm running water after toileting or diapering.
Evidence: The provider changed the diaper of two children, one after the other. The children's hands were not washed after diapering.

Plan of Correction: Since the observation, I have been washing the children' hands after their diaper change
