Comments:

An unannounced renewal inspection was conducted today from approximately 9:45 am to 11:00 am. There were 4 children in care of the provider. A walk-through of the entire home was completed. 3 children?s, 1 staff record, 1 household member record, caregiver training, children?s nutrition, supplies, emergency procedures and drills were inspected. The children were observed napping. There were age appropriate materials for all children in care. Areas of non-compliance are identified in the Violation Notice.

Description:	Based on children's record review, two out of six records did not contain results of the health examination for the child. Evidence: 1. Child #2 began care on 12/29/2016, no health exam was found for this child. 2. Child #4 began care on 12/12/2017, no health exam found for this child.
