Comments:
An unannounced monitoring inspection was conducted on 6/20/19 and 10/2/19, in conjunction with the local department of social services, to investigate a self-reported incident regarding the death of a child in care. Provider interviews were conducted, documentation and photos were reviewed and observations were made. Violations were issued as a result of this inspection. Violations:
Standard #: 22VAC40-111-140-B-1
Description: Based on a review of documentation, it was determined that not all substitute providers had high school completion or the equivalent. Evidence: Caregiver #2, who acted as a substitute provider on 10/1/19, did not have documentation of a high school diploma or equivalent in their file.

Plan of Correction: Caregiver #2 high school diploma provided to inspector on 11/20/19. Will ensure caregiver records are complete in the future.

Standard #: 22VAC40-111-140-D
Description: Based on a review of documentation, and provider interviews, the provider failed to ensure that a substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works. Evidence: on 10/1/19, caregiver #2 acted as a substitute provider from approximately 8:30 -5:00 pm and there is not a signed time sheet for this day.

Plan of Correction: Training was provided, and procedures reinforced to ensure daily entries on time sheets are logged. Reinforcement of administrative processes will be conducted on a monthly basis.

Standard #: 22VAC40-111-530-B
Description: Based on observation, interviews and a review of photos, it was determined that not all cribs used by infants had a tight-fitting bottom cover. Evidence: On June 17, 2019, two cribs being used by infants had sheets that were not tight fitting and allowed material to gather on the surface of the mattress presenting a suffocation or strangulation hazard.

Plan of Correction: Elastic straps were added to the sheets to ensure a tight fit is maintained. Elastic straps check will be added to the weekly daycare checklist to ensure serviceability and effectiveness is maintained.

Standard #: 22VAC40-111-530-F
Description: Based on observation, interviews and a review of photos, the provider failed to ensure that soft bedding of any kind was not used under or around sleeping infants. Evidence: On June 17, 2019, a four month old infant was sleeping in a crib with a receiving blanket and a stuffed animal toy inside the crib. There was also a crocheted blanket hanging over the side of the crib.

Plan of Correction: Training was provided to ensure no soft items are in, near, or around the crib of sleeping infants. Refresher training and reinforcement will be conducted on a monthly basis.

Standard #: 22VAC40-111-560-A
Description: Based on interviews, it was determined that the provider did not ensure direct care and supervision of each child to include monitoring of each sleeping infant by placing each infant to sleep in a location where the infant is within sight and hearing of a caregiver; by in-person observation of each sleeping infant at least once every 15 minutes; or by using a baby monitor. Evidence: 1. Caregiver #2 stated that they check on sleeping infants every 15-20 minutes. 2. Caregiver #1 stated that they check on sleeping infants every 30 minutes, but sometimes check more frequently if they are using the restroom or heating up food in the microwave oven. 3. Both caregivers stated that they do not use a baby monitor.

Plan of Correction: Training was provided to ensure sleeping infants are monitored every 15 minutes. We will continue to monitor babies via monitor or physically every 15 minutes. Refresher training and reinforcement will be conducted on a monthly basis.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of documentation, and provider interview, the provider did not submit a central registry search within 30 days of a person in the home becoming 14 years old. Evidence: Household member C, (date of birth: 3/22/2005), did not have documentation of a completed central registry search in their file.

Plan of Correction: A Central Registry Search was submitted for my 14-year old son on 11/20/19. Continuous records checks will ensure this type of oversight is reduced in the future.
