An out of state background check must be completed for all caregivers, that have lived out of state within the past five years. You can access the information on www.dss.va.gov, search "Adam Walsh out of state" and look for the state you need. Complete the form and send out, send results to inspector within 30 days of this notice.
An unannounced monitoring inspection was conducted today from 9:00am-11:30am. There were 6 children (15 points) supervised by 3 direct caregivers. The home, 2 caregiver records, 5 children?s records, 1 provider record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed eating snack, working on numbers, circle time, singing and dancing. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-750-A Description: Based on observation, topical skin products are not labeled with chid's name.
There are two topical skin products by the diaper changing area that is not labeled with child's name.
Plan of Correction: The staff use that for our hands, I'll remove it.
Standard #: 22VAC40-111-120-A Description: Based on review of two caregiver record's two are incomplete.
Caregiver #1 does not have documentation of orientation, emergency contacts, reference checks, and statement of health.
Caregiver #2 does not have documentation of emergency contacts, one reference check or proof of age.
Plan of Correction: I will review their records and get all the documentation I need.
Standard #: 22VAC40-111-690-D Description: Based on discussion with provider, the diapering surface is not being cleaned and sanitized according to licensing standards.
Provider stated the diapering surface is being cleaned with a solution of bleach, soap and water. Licensing standards require the diapering surface be sprayed with a solution of soap & water, patted dry, sprayed with a solution of bleach & water and allowed to air dry after each diaper change.
Plan of Correction: I will begin using that method.
Standard #: 63.2-1720.1-B-3 Description: Based on review of two caregiver record's, one does not have documentation of an out of state background check for a caregiver who lived out of state within the past five years.
Caregiver # 1 has lived out of state within the past five years and has not had an out of state central registry check completed within 30 days of employment.
Plan of Correction: I didn't know I needed to do that, I will send it in and email you the results when they are received.
