An unannounced monitoring inspection was conducted from 11:45 am - 1:15 pm with the provider and her assistant. There were 11 daycare children and one resident child in care, ranging from ten months to five years of age, for a total of 28 points. The children were observed playing outside, coming inside and washing hands, eating lunch and preparing for nap. Lunch served today was chicken salad, crackers, carrots, fresh pineapple and milk. Four children's records were reviewed. Three caregiver/household member records were reviewed and in compliance. Required information was posted. The last emergency escape drill was conducted: 11/19/19 and the last shelter-in-place drill was conducted: 10/23/19. The provider has current certification in first aid/CPR and Emergency Medication Administration Training (EMAT) and one medication was reviewed and in compliance. Standard #: 22VAC40-111-100-A Description: Based on a review of four children's records, the provider did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: The record for child #4 (date of first attendance: 6/24/19) did not have documentation of a physical examination. Plan of Correction: Will ask the parent for the missing documentation.
