Comments:
A renewal inspection was conducted today from 11:15-12:15. No children were in care during the inspection. The provider is currently using only the upstairs area as enrollment is low. Reviewed one child's record, background checks, physical space, emergency preparation and trainings. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on record review, a household member (HHM) did not have a current TB screening.
Evidence: HHM #1's TB report was older than 2 years (2016)

Plan of Correction: The household member obtained a new TB screening test.

Standard #: 22VAC40-111-210-A
Description: Based on provider statement, no documentation was available to verify that the provider had obtained the required training the past licensure year.
Evidence:
The provider was unable to provide documentation that she had obtained the required 16 hours of annual training.

Plan of Correction: The provider scanned certificates to the Licensing Inspector to verify hours of annual training. Provider obtained 21 hours over the past licensure year. Certificates were sent by text on 11/21/2019.

Standard #: 22VAC40-111-240-A
Description: Based on observation, an area of the home was not maintained in a safe or operable condition.
Evidence:
The door handle for the storm door leading into the home was broken off.
