Comments:
An unannounced renewal inspection was conducted with the provider on November 22, 2019 to determine compliance with the licensing standards. There were four day care children present, ranging in age from 11 months to 2 years, totaling 12 points. The children were having rest time. Lunch served today: ravioli, corn, crackers, fruit with jello, and milk. Two new children's files and the provider's file were reviewed - see Supplemental Summary for details. Provider does not administer medication - Medication Administration Training (MAT) is required to administer medicine to day care children. First aid and CPR certification was renewed 6/2019. Emergency evacuations have been conducted monthly; the last shelter in place drill was conducted 10/4/2019. Time of today's inspection: 11:00 a.m. to 12:30 p.m.

Standard #: 22VAC40-111-100-A
Description: Based on review of two children's files, the provider failed to have documentation of a physical examination within 30 days after the first day of attendance. Evidence: Child A (date of enrollment 8/2019) and Child B (date of enrollment 10/14/2019) did not have documentation of the child's physical on file.

Plan of Correction: Provider will ask the parents for the physical for the file.

Standard #: 22VAC40-111-60-B
Description: Based on review of two children's files, the provider failed to have all of the required information in each child's file. Evidence: Child B did not have documentation of the addresses for two emergency contacts and did not have the address and phone number for the child's physician.

Plan of Correction: Provider will request the information from the parent again.

Standard #: 22VAC40-111-180-A
Description: Based on review of the provider's file, the provider failed to have documentation of a repeat tuberculosis test or screening within two years of the previous test or screening.

Plan of Correction: Provider will schedule an appointment for a tuberculosis test or screening.
