Comments:
An unannounced mandated Renewal inspection was conducted today. There were 2 children present (6 points) with 2 caregivers. Children were observed napping, drinking a bottle, playing with stuffed animals, an activity center and manipulative toys. Reviewed 2 children's files, 2 caregiver files, 3 resident files, attendance, first aid kit, menus, emergency evacuation drills and plan, crib compliance, and physical plant. There were no medications on site. Violations of the Standards have been recorded on the Violation Notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child#1 (start date 2/11/19) and Child#2 (start date 10/2019) did not have the name, telephone and address for emergency contacts incase the parents can not be reached. Child#1 was missing 1 contact, Child#2 was missing both,
2. Child#2 was missing the policy number of the child's medical insurance carrier.

Plan of Correction: I will get the information from the parents.

Standard #: 22VAC40-111-70-A
Description: Based on record review, parent's were not given the family day care homes policies prior to the first day of care.
Evidence:
Child#1 (start date 10/2019) did not have documentation of receiving the home's medication administration policy.

Plan of Correction: I will give it to the parent for signature

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized prior to the child's first day.
Evidence:
Child#2 (start date 2/11/19) did not have documentation of immunizations.

Plan of Correction: I thought the parent gave them to me, but I will get another copy.
