An unannounced monitoring inspection was completed on 11/20/2019. There were four children in care with the provider and a household member/assistant supervising at a total of nine points. Four children's records, the provider's record and one household member's (assistant) record were reviewed. The inspector arrived at the provider's home at 1:20 P.M. and departed at 2:45 P.M.
The children were observed during free play times, nap and TV time.
There was discussion with the provider about the following: annual training, fire drill happening at different time of day and emergency contacts.

Standard #: 22VAC40-111-180-A Description: Based on review of the records, the provider failed to ensure that all repeat TB screenings were completed every two years.

1. Provider's and household member's most recent TB screening and or test was 10/6/2017Plan of Correction: The provider and house hold member will go Monday and will monitor all time frames.
