63.2 Licensure and Registration Procedures

Technical Assistance:
Technical Assistance was provided in regards of clarification of what is required for emergency drills, background checks and the assistant's documentation requirements. The provider must send to the licensing inspector the background checks results as soon as they are available.

Comments:
An unannounced monitoring visit was conducted today between the hours of approximately 9:30 am through 12:00pm. At the time of entrance, there were 2 children (8 points) with the provider. A sample of 2 children's files and the provider's files were reviewed. The children were engaged in the following: toy play, puzzles,
counting with manipulatives, letter recognition with flashcards, story reading with the provider, drawing and coloring and other organized activities. The provider was observed guiding the children through the learning activities and providing positive reinforcement and redirecting when necessary. The physical space of the care area, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. There were no medications on site today. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-770
Description: Based on emergency supplies review and interview, it was determined that a a working portable battery-operated weather band radio was not available to caregivers at all times.
Evidence:
1) The weather band radio did not have working batteries so it did not turn on.
2) The provider tried to locate additional batteries but could not find more.

Plan of Correction: I will get new batteries.

Standard #: 22VAC40-111-830-A
Description: Based on documentation review, it was determined that the emergency evacuation procedures were not practiced monthly.
Evidence:
1) Evacuation drills documented for the September 2018 - May 2019 period did not document evacuation drills for March 2019.
2) Evacuation drills documented for the September 2019 - November 2019 period did not document evacuation drills for October 2019.

Plan of Correction: I will complete one evacuation drill monthly.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain address and phone number for the place of employment for both parents, a medical insurance policy number and the date of the first day of attendance.

Plan of Correction: Will ask mom to complete the information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not provided in writing with all required information.
Evidence:
1) The record for Child #2 did not contain the complete policies for the administration of medications for the family day home (FDH). The record contained page 2 of the document but was missing page 1 where the FDH policy is explained.

Plan of Correction: Will ask mom to complete the information.

Standard #: 22VAC40-111-320-G
Description: Based on document review and interview, it was determined that the fireplaces in the family day home (FDH) were not inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
1) The provider stated that there are two fireplaces in the FDH: one in the living room and one in the basement. The provider stated that because they are not used, they haven't been inspected.
2) The most recent fireplace inspection report available for review was dated 04/16/13.

Plan of Correction: The appointment is set for December 5th, 2019.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, it was determined that the family day system home did not have a central registry finding within 30 days of employment.
Evidence;
1) The record for Staff #2 documents a hire date of 10/06/2019. There is no documentation available of a central registry finding available for review.
2) The provider stated that she thought it had been sent.

Plan of Correction: I will send the documentation today.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and interview, it was determined that the family day home did not have a criminal history records check prior to the first day of employment.
Evidence;
1) The record for Staff #2 documents a hire date of 10/06/2019. There is no documentation available of a criminal history records check.
2) The provider stated that they have not set an appointment date for the criminal history check.

Plan of Correction: I will set the appointment as soon as possible.
