Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, children records, and fire safety and shock and prevention.

An unannounced renewal inspection was conducted today from approximately 11:00 am to 12:35 pm. There were 4 children (9 points) in care of the provider.

2 children?s, 1 staff record, 1 household member record, caregiver training, children?s nutrition, supplies, emergency procedures and drills were inspected. The children were observed playing freely and with toys on the floor. There were age appropriate materials for all children in care. Hand washing was observed. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-240-A
Description: Based on observation and staff interview, areas and furnishings of the family day home, inside were not maintained in a clean, safe, and operable condition.
1. Two mats used for children for nap time had torn material on the sides that were accessible to children.

Plan of Correction: I have purchased new sleeping mats and threw away the old ones.

Standard #: 22VAC40-111-430-B
Description: Based on observation, the provider used a pressure mounted gate as a protective barrier at stair openings.
1.The bottom of the steps leading to the second floor inside the home were blocked with an pressure mounted gate.
2. The gate was not mounted.
3. Children under two years of age were in care during the inspection and were in the indoor play area by the gate.

Plan of Correction: I have purchased two new hard mounted gates. They will be installed this week. I will no longer use the pressure mounted gate.

Standard #: 22VAC40-111-430-C
Description: Based on observation, the provider did not ensure children over the age of two did not have access to stairs with three or more risers that do not have protective barriers or guardrails on each side.
1. On the date of inspection (11/12/19), a protective barrier was not installed at the bottom of the steps, located on the main level, to prevent children from having access to stairs with three or more risers.
2. Children over the age of two were in care during the inspection and in the indoor play area on the main level of the home.

Plan of Correction: I have purchased new gates and they will be installed this week.

Standard #: 22VAC40-111-690-D
Description: Based on observation and staff interview, the provider did not ensure the diapering surface was nonabsorbent and washable.
Evidence: The diapering changing surface had torn material in the corners of the changing surface.

Plan of Correction: I have purchased a new changing pad for diapering changing.
