Comments:
An unannounced monitoring inspection was completed today. The physical plant, 5 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:00-2:15pm. There were 7 children, totaling 16 points, under the supervision of 2 caregivers. Today, the children learned about farms and Thanksgiving, worked on craft projects, participated in circle time, ate lunch and rested at nap time. Today, the children were served macaroni and cheese,pasta with tomato sauce and cheese, carrots and peas, peaches and milk. Standard #: 22VAC40-111-700-A
Description: Based on file review, the Provider did not obtain a written authorization from a parent to administer a medication for a child in care.
Evidence
1. Child #4 has an epi-pen at the FDH in case of an emergency due to food allergy. There is no written authorization from the parent, which would allow the Provider to administer the medication.

Plan of Correction: The LI printed a medication form, which the Provider will give to the parent to have completed.

Standard #: 22VAC40-111-100-A
Description: Based on file review, 1 out of 5 children's records does not have a physical exam within 30 days after the first day of attendance.
Evidence
1. Child #5 began care on 9/16/19. There is no physical on file

Plan of Correction: The Provider will ask the parents for this.

Standard #: 22VAC40-111-90-C
Description: Based on file review, 2 out of 5 children's records do not have additional immunizations every six months up to the age of 2 years.
Evidence
1. Child #3 is 23 months old. The most recent immunization record on file was completed when the child was 3 months old.
2. Child #5 is 2 years old. The most recent immunization record on file was completed when the child was 6 months old.

Plan of Correction: The Provider will ask the parents for these.
