Comments:
A Subsidy Health and Safety inspection was conducted today at 1:00pm to review supplemental health and safety requirements. There were 7 children under the supervision of 2 staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. There are no children receiving subsidy at this time. Standard #: 22VAC40-665-170-B-12
Description: Based on file review, the Provider does not have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence
1. Child #3 requires an epi-pen on site in case of an emergency due to a food allergy. There is no written care plan from a physician, including the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The Provider will ask the parent to get this from the child's physician.
