Technical Assistance:
Technical Assistance was offered regarding background checks.

Comments:
An unannounced renewal inspection was conducted from 11:00 am to 12:15 pm. There were 11 daycare children and 1 resident child (31 points) present with two caregivers. The children were observed paying in small groups with a variety of age appropriate toys. There were no medications on site. A complete walk through of the family day home was conducted. 5 children's records, 2 caregiver records and 1 household member record was reviewed. Emergency drill records were current. Standard #: 22VAC40-191-40-D-1-c
Description: Based on records review, the family day home did not obtain current sworn disclosure statements (SDS) for adult household members and employees, every five years.
Evidence:
1. Caregiver # 1 and Caregiver # 2's most recent SDS expired on 4/7/19.
2. Household member # 1's most recent SDS expired on 4/7/19.

Plan of Correction: All three forms were updated during the inspection.
