Technical Assistance:
Reviewed topics covered in orientation training. The provider was encouraged to use the model form available on the DSS website.

Comments:
Today an unannounced Renewal inspection took place between 9:05am and 12:00pm. There were 8 children present with 1 of the providers children with the provider and 2 assistant for a total of 19 points within the supervision guidelines. The children were observed playing freely, using legs, looking at books and doing a Thanksgiving art project. Diaper changing and hand washing procedures were observed. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Thank you for your cooperation. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit did not contain the minimum supplies.
Evidence: The first aid kit does not contain two triangular bandages.

Plan of Correction: I'll get another one next week.

Standard #: 22VAC40-111-890
Description: Based on observation, meals and snack brought from home were not clearly labeled in a way that identifies the owner.
Evidence: Three out of six children's lunch boxes stored in the providers refrigerator were not labeled with a child's name.

Plan of Correction: I'll remind the parents that they need to do it.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider has not obtained updated immunizations every six months for children under the age of 2 years.
Evidence: Child #2 (14 months) most recent documented immunizations are from when the child was 2 months old.

Plan of Correction: I'll text the mother and I'll have her bring it next week.

Standard #: 22VAC40-111-200-A
Description: Based on record review and interview, the provider has not orientated each assistant by the end of their first week assuming job responsibilities.
Evidence: Caregiver #3 (date of hire: 8/20/18) file does not contain documentation of orientation training. When reviewing the topics covered in orientation training, the provider confirmed that she has not completed this training with this assistant caregiver.

Plan of Correction: I'll print form from the DSS website and do the training tomorrow.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the family day home are not maintained in a clean, safe and operable condition.
Evidence: There is an area of approximately 2" X 2" of chipped and peeling paint behind the swinging chair. There is an area of approximately 3" X 3" of chipped and peeling paint behind the table where the children eat.

Plan of Correction: My husband can paint it.

Standard #: 22VAC40-111-680-A
Description: Based on observation, caregivers did not wash their hands after coming in contact with bodily fluids.
Evidence: Caregiver #1 was observed wiping the nose of child #6. The caregiver did not wash their hands after wiping the child's nose.

Plan of Correction: I'll make sure I'm doing it.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed after diapering.
Evidence: Child #1 was observed having their diaper changed. The child's hands were not washed after their diaper change.

Plan of Correction: I'll tell her now.
